Results from brand semaglutide trials
Our pick

01  Lower risk of heart attack & stroke

Heart disease plus excess weight

02  Better blood sugar & A1C

Type 2 diabetes, the approved use

1.6point A1C drop on Ozempic 1 mg, vs 0.1 on placebo

03  Lower blood pressure

A measured bonus, not a hypertension treatment

6.2mmHg systolic drop vs 1.1 on placebo, 68 weeks

Compounded semaglutide was not in these trials. Every result below comes from brand semaglutide, sold as Ozempic or Wegovy. Compounded semaglutide is not FDA approved, and the FDA does not review it for safety, effectiveness or quality before it is sold.

Semaglutide benefits at a glance

Semaglutide is a GLP-1 medicine, most often taken as a weekly injection, that acts like a hormone your gut releases after meals. The best-known semaglutide benefits are appetite and weight, explained in how semaglutide works for weight. Large trials have also measured what it does for the heart, kidneys, liver, joints and blood work.

The table lists nine semaglutide health benefits, the trial behind each and whether the FDA has approved semaglutide for that use. Four carry an approval. Five are trial findings only.

Approval years from Drugs@FDA. Semaglutide 2.4 mg weekly is the usual Wegovy injection maintenance dose. Those trials studied uses the label does not cover.

Health benefits of semaglutide backed by trials

Below are the health benefits of semaglutide with trial data behind them. Averages describe groups, and individual responses vary.

Two caveats apply throughout. Semaglutide's maker funded most of these trials, and every one used brand semaglutide, not compounded semaglutide, which is not FDA approved and was not studied in them.

01

Lower risk of heart attack & stroke

SELECT and SUSTAIN 6, both FDA approved uses

20%lower risk of major heart events in SELECT

SELECT enrolled 17,604 adults with heart disease and overweight or obesity but no diabetes. Over a mean 39.8 months, a heart attack, stroke or heart death struck 6.5% on semaglutide 2.4 mg and 8.0% on placebo.

Where it wins

  • The FDA approval for Wegovy heart risk came on March 8, 2024, the first for a weight loss medication
  • Ozempic carries a heart indication in type 2 diabetes with heart disease, based on SUSTAIN 6 (6.6% vs 8.9%)
  • Both results were on top of standard heart care

What it costs you

  • Approved only for adults with established heart disease
  • The Wegovy label says an effect on heart failure has not been established
  • Compounded semaglutide was not tested

Heart disease plus excess weight

02

Better blood sugar & A1C

Ozempic, SUSTAIN 1, FDA approved 2017

1.6point A1C drop on Ozempic 1 mg, vs 0.1 on placebo

A1C shows your average blood sugar over the past two to three months. Ozempic was approved on December 5, 2017 for blood sugar control in adults with type 2 diabetes. In the 30-week trial on its label, A1C fell 1.4 points on 0.5 mg and 1.6 on 1 mg, from about 8.1%, against 0.1 on placebo.

Where it wins

  • 70% to 73% reached an A1C under 7%, vs 28% on placebo
  • Fasting blood sugar fell more too

What it costs you

  • The approval covers type 2 diabetes only
  • In STEP 1, adults without diabetes started near 5.7% and dropped 0.4 points vs 0.2 on placebo

Type 2 diabetes, the approved use

03

Lower blood pressure

STEP 1 data on the Wegovy label

6.2mmHg systolic drop vs 1.1 on placebo, 68 weeks

STEP 1 followed 1,961 adults with excess weight and no diabetes for 68 weeks. Systolic pressure, the top number, fell 6.2 mmHg on semaglutide 2.4 mg and 1.1 mmHg on placebo, from a starting average near 127.

Where it wins

  • Semaglutide blood pressure results appear in the Wegovy label's table of heart risk factors
  • Diastolic pressure, the bottom number, fell 2.8 mmHg vs 0.4

What it costs you

  • The systolic drop was a formally tested goal of the trial. The diastolic drop was not
  • Keep taking your blood pressure medicine unless your prescriber changes it

A measured bonus, not a hypertension treatment

04

Healthier cholesterol & triglycerides

Lipid results from STEP 1, not an approved use

21.9%triglyceride drop vs 7.3% on placebo

In STEP 1, triglycerides fell 21.9% on semaglutide 2.4 mg vs 7.3% on placebo. HDL, the protective cholesterol, rose 5.2% vs 1.4%.

Where it wins

  • The largest lipid change in the label table
  • Total cholesterol fell 3.3% vs a 0.1% rise

What it costs you

  • LDL, the cholesterol statins target, changed little: down 2.5% vs up 1.3%
  • Not a formally tested goal, and not a replacement for a statin

Triglycerides more than LDL

05

Quieter food noise

Appetite trial at the 2.4 mg Wegovy dose

35%less eaten at a free-choice lunch vs placebo

Food noise is an informal term for constant thoughts about food. No trial measures it by that name. In a 20-week trial of 72 adults with obesity, people on semaglutide 2.4 mg ate 35% less than placebo at a lunch where they could eat freely.

Where it wins

  • Fewer, weaker cravings on a standard eating questionnaire
  • Better control of eating, less hunger and more fullness than placebo

What it costs you

  • A small trial of 72 people
  • Food noise still has no agreed definition

Cravings and control of eating

06

Kidney protection in diabetes

FLOW trial, Ozempic, FDA approved 2025

24%lower risk of major kidney events

FLOW enrolled 3,533 adults with type 2 diabetes and chronic kidney disease. Over a median 3.4 years, Ozempic 1 mg cut the risk of kidney failure, a 50% loss of kidney function, or kidney or heart death by 24%. It stopped early after a planned interim check.

Where it wins

  • FDA approved for this use on January 28, 2025
  • Heart deaths were also lower, 7.0% vs 9.6%

What it costs you

  • Studied only in type 2 diabetes with kidney disease
  • Nearly all participants already took standard kidney-protecting medicines

Diabetes with chronic kidney disease

07

Less liver fat

ESSENCE trial, Wegovy, accelerated approval 2025

63%MASH resolved vs 34% on placebo, 72 weeks

MASH is fatty liver disease that has progressed to inflammation and liver cell damage. ESSENCE biopsied 800 adults with MASH and moderate to advanced scarring after 72 weeks on semaglutide 2.4 mg or placebo. The inflammation resolved without worse scarring in 63% vs 34%, and scarring improved in 37% vs 22%.

Where it wins

  • FDA accelerated approval for Wegovy in MASH without cirrhosis came on August 15, 2025
  • A separate 72-week MRI trial measured liver fat directly and found larger drops than placebo

What it costs you

  • Accelerated approval means a confirmatory trial still has to show clinical benefit
  • The MRI trial used a daily 0.4 mg dose that no marketed product uses, and its main goal, liver stiffness, did not differ from placebo
  • Not approved for cirrhosis

MASH with moderate to advanced scarring

08

Less knee arthritis pain

STEP 9 at the 2.4 mg Wegovy dose

41.7point drop in knee pain score vs 27.5 on placebo

STEP 9 enrolled 407 adults with obesity and moderate knee osteoarthritis. After 68 weeks, the 0 to 100 WOMAC knee pain score fell 41.7 points on semaglutide 2.4 mg vs 27.5 on placebo, with weight down 13.7% vs 3.2%.

Where it wins

  • Function scores improved more, 12.0 vs 6.5 points
  • Both groups got diet and activity counseling

What it costs you

  • No FDA approval for osteoarthritis
  • Pain fell on placebo too. The gap is the drug's added effect

Obesity with knee osteoarthritis

09

Lower inflammation markers

CRP results from STEP-HFpEF at 2.4 mg

43.5%drop in CRP vs 7.3% on placebo, 52 weeks

Your liver makes C-reactive protein, or CRP, in response to inflammation. In STEP-HFpEF, 529 adults with obesity and a type of heart failure called HFpEF took semaglutide 2.4 mg or placebo for 52 weeks. CRP fell 43.5% vs 7.3%.

Where it wins

  • CRP was a formally tested goal of the trial
  • Heart failure symptom scores also improved, 16.6 vs 8.7 points

What it costs you

  • A lower lab marker does not mean fewer heart events
  • No FDA approval for inflammation or heart failure

A lab signal, not a reason to start on its own

How to choose a semaglutide option

Every benefit above comes from Ozempic or Wegovy, which sets up the first choice: brand or compounded.

  • Ozempic benefits on its label: blood sugar control in type 2 diabetes, plus lower heart risk with heart disease and kidney risk with kidney disease. It is not approved for weight management.
  • Wegovy benefits on its label: long-term weight management, lower heart event risk with heart disease, and MASH.
  • Compounded semaglutide is prepared by a licensed pharmacy for an individual patient. It is not FDA approved, and no trial above tested it.

If a labeled use fits you and insurance covers the brand, the brand carries the proof. For cash-pay weight management, Arbor compounded semaglutide starts at $99 a month and stays at one price as the dose rises. Through Arbor, a board-certified physician reviews your health history first. If it is not the right fit, you are not charged.

The semaglutide injection benefits here are trial averages, not a promise. Your clinician sets the dose and how fast it rises.

Who should not take semaglutide

Brand semaglutide carries a boxed warning and is not for anyone with a personal or family history of medullary thyroid carcinoma or MEN 2, or a prior serious allergic reaction to semaglutide. The Wegovy label says to stop it once pregnancy is recognized when it is used for weight loss or heart risk reduction. A clinician checks these before prescribing. For anything about your own history, message your Arbor clinician.

Frequently asked questions

Asking what are the benefits of semaglutide besides weight usually means the heart, kidneys and blood work. Brand semaglutide is FDA approved to lower heart event risk, control A1C in type 2 diabetes, slow kidney decline in diabetic kidney disease and treat MASH. Trials also showed lower blood pressure, triglycerides, cravings, knee pain and CRP, with no approval for those.

Yes, for specific groups, in large trials. Ozempic heart benefits come from SUSTAIN 6 in adults with type 2 diabetes and heart disease, and Wegovy lowered major heart event risk by 20% in SELECT. Both are labeled uses.

No. Wegovy is an FDA approved brand drug. Compounded semaglutide is not FDA approved, and the FDA does not review compounded drugs for safety, effectiveness or quality before sale. The FDA declared the semaglutide shortage resolved on February 21, 2025. The semaglutide benefits above come from Ozempic and Wegovy trials, and none tested a compounded product.
