The verdict: The weekly semaglutide shot produced more average weight loss in its trial, needs one dose a week, and costs $89 a month through Arbor. Foundayo is an FDA-approved pill with no needle. Arbor's semaglutide is compounded and not FDA-approved.

Semaglutide shots vs Foundayo verdict

On the numbers that decide most choices, semaglutide shots lead. In STEP 1, adults without diabetes on weekly semaglutide 2.4 mg lost an average of 14.9% of their body weight over 68 weeks, against 2.4% on placebo (NEJM, 2021). In ATTAIN-1, adults on the top dose of Foundayo lost 11.1% over 72 weeks, against 2.1% (Foundayo prescribing information).

No trial has measured orforglipron vs semaglutide injections for weight loss, so treat that gap as a direction only. The shot also takes one dose a week instead of seven. Foundayo has real advantages: no needle, FDA approval in April 2026, and tablets kept at room temperature.

The table answers the “glp 1 pill vs shot” question row by row, from the FDA labels and launch pricing. Wegovy is the brand of weekly semaglutide for weight management, and STEP 1 tested its usual 2.4 mg dose, so the semaglutide column also answers foundayo vs wegovy. To compare the two forms of semaglutide, see the Wegovy pill vs Wegovy injection guide.

Semaglutide vs Foundayo side by side

How semaglutide & orforglipron work

Both drugs act on the GLP-1 receptor, which helps govern appetite and fullness. Semaglutide is a peptide, a modified copy of the gut hormone GLP-1 with a long half-life, which is why it is given as a once-weekly injection. The guide to how semaglutide works for weight loss has the detail.

Foundayo is the brand name for orforglipron, a small-molecule, nonpeptide GLP-1 receptor agonist designed to be swallowed (NEJM, 2025). FDA’s approval announcement called it a GLP-1 receptor partial agonist pill that does not need to be taken on an empty stomach.

Weight loss results in trials

ATTAIN-1 enrolled 3,127 adults with obesity, or overweight plus a related condition, and no diabetes. The ATTAIN-1 trial on ClinicalTrials.gov lists the design: randomized, double-blind and placebo-controlled for 72 weeks. On the label, orforglipron weight loss averaged 7.4% on 5.5 mg, 8.3% on 9 mg and 11.1% on 17.2 mg, against 2.1% on placebo. At the top dose, 54.5% lost at least 10% of their weight and 18.4% lost at least 20%.

STEP 1 enrolled 1,961 adults without diabetes for 68 weeks. On the Wegovy label, 66.1% of people on semaglutide 2.4 mg lost at least 10% and 30.2% lost at least 20%, against 12% and 1.7% on placebo (Wegovy prescribing information).

These are separate trials of different lengths and populations, both funded by the drugmakers. The one head-to-head study, ACHIEVE-3, pitted orforglipron against oral semaglutide at diabetes doses in adults with type 2 diabetes. Orforglipron lowered blood sugar more, but weight was not the main measure and injectable semaglutide was not tested (Lancet, 2026). Dosing & daily routine

Foundayo dosage climbs in monthly steps. The label starts at 0.8 mg once daily, moves to 2.5 mg after at least 30 days, then 5.5 mg, and can rise to 9 mg, 14.5 mg or 17.2 mg, with at least 30 days at each level. You take one tablet a day, with or without food, with no water limit or wait.

Weekly semaglutide on the Wegovy label starts at 0.25 mg and rises every 4 weeks to a usual 2.4 mg maintenance dose. It is injected on the same day each week, at any time, with or without meals. For compounded semaglutide, your Arbor clinician sets the dose and schedule.

The real difference is routine. On the Wegovy label, a missed injection can still be given if the next one is more than 2 days away. Foundayo’s label says to take a missed tablet as soon as possible, without doubling up.

Foundayo cost vs semaglutide cost

At approval in April 2026, CNBC reported Foundayo cost as $149 to $349 out of pocket, depending on the dose, or $25 a month with insurance coverage and a coupon from the maker (CNBC, April 1, 2026). The White House had announced in November 2025 that if FDA approved an oral GLP-1 of this kind, its initial dose would be priced at $150 a month through TrumpRx (White House, 2025). Manufacturer prices change often.

Arbor lists compounded semaglutide at $89 a month, and the price does not rise as your dose goes up, which matters most once you reach a maintenance dose.

FDA status & access

FDA approved Foundayo on April 1, 2026, 50 days after filing, under its National Priority Voucher pilot program (FDA). Its approved use is to reduce excess body weight and keep it off long term, in adults with obesity or with overweight and a related condition. Weekly semaglutide has been FDA-approved for weight management as Wegovy since 2021.

Compounded semaglutide is not FDA-approved, and FDA does not review compounded drugs for safety, effectiveness or quality before they reach patients (FDA). FDA declared the semaglutide shortage resolved in February 2025, so compounding now runs on individual prescriptions from a licensed pharmacy (FDA). Every trial result in this article comes from the brand products.

Who should choose semaglutide shots or Foundayo

Arbor compounded semaglutide is a weekly injection at a flat monthly price, and a licensed clinician first decides whether it fits your goals and history. Foundayo is not an Arbor program.

A weekly semaglutide shot may suit you when:

  • The size of the average result matters most to you
  • You would rather handle treatment once a week than every day
  • You want a single monthly price that stays flat as your dose rises
  • You take oral birth control, since Foundayo’s label asks for a backup method for 30 days after starting and after each dose increase

Foundayo may suit you when:

  • Needles are the reason you have not started treatment
  • You want an FDA-approved brand product rather than a compounded one
  • You travel often and want medicine that keeps at room temperature

Neither drug is prescribed to anyone with a personal or family history of medullary thyroid carcinoma or MEN 2, and both are stopped in pregnancy. Foundayo also interacts with some medicines that act on the liver enzyme CYP3A4, which a clinician checks. For anything else, message your Arbor clinician.

Bottom line on a weekly shot vs a daily pill

The weekly shot carries the larger trial result and the lighter routine, at one monthly price through Arbor. Foundayo fits better when a needle is the barrier, or when an FDA-approved brand matters more than the size of the effect.

That same logic settles foundayo vs ozempic. Ozempic is weekly semaglutide too, approved for type 2 diabetes rather than weight management (Ozempic prescribing information). A licensed clinician reviews your history before either treatment.

Arbor is a technology platform. Medical evaluations are conducted by independent, licensed healthcare providers who decide whether treatment is appropriate. Compounded medications are prepared by licensed U.S. pharmacies for individual patients and are not reviewed by the FDA for safety or effectiveness.

Frequently asked questions

Orforglipron activates the GLP-1 receptor, which its label says lowers appetite and food intake and slows stomach emptying. As a small molecule, not a peptide, it can be swallowed as a once-daily tablet.

Wegovy injection at 2.4 mg produced 14.9% average weight loss at 68 weeks and Foundayo 11.1% at 72 weeks, in separate trials. The Wegovy pill reached 13.6% at 64 weeks but must be taken on an empty stomach with no more than 4 ounces of water, then a 30-minute wait. Wegovy also carries a heart-risk indication that Foundayo does not.

No. Foundayo is orforglipron, a different molecule sold as an FDA-approved brand tablet. Compounded semaglutide is prepared by a licensed pharmacy for one patient and is not FDA-approved, nor is it the same product as Wegovy.
