The verdict: Zepbound produced more weight loss than Wegovy in the one head-to-head trial. Wegovy carries a proven heart benefit, a pill form and more approved uses. Arbor prescribes compounded tirzepatide and compounded semaglutide, which are not FDA-approved products.

Zepbound vs Wegovy verdict

If the question is wegovy vs zepbound on the scale, the answer is Zepbound. In SURMOUNT-5, adults with obesity and no diabetes were randomized to the highest dose of either drug they could tolerate. After 72 weeks, average weight change was 20.2% on tirzepatide, the drug in Zepbound, and 13.7% on semaglutide, the drug in Wegovy (NEJM, 2025).

That settles zepbound vs wegovy weight loss at the doses tested, not the whole decision. Wegovy holds a cardiovascular outcomes indication that Zepbound does not, comes as a daily pill as well as a weekly shot, and can cost less in cash. Zepbound is also approved for obstructive sleep apnea in adults with obesity.

Tirzepatide vs semaglutide side by side

Underneath the brand names, this is tirzepatide vs semaglutide. Semaglutide acts on one gut hormone receptor, GLP-1. Tirzepatide adds a second, GIP. Both slow stomach emptying and quiet appetite, and both injectables are weekly.

Weight loss results in trials

SURMOUNT-5 is the only trial built to answer tirzepatide vs semaglutide for weight loss directly. It randomized 751 adults, and those on tirzepatide were more likely to reach every threshold measured, from 10% to 25% of body weight lost. Waist size fell 18.4 cm against 13.0 cm. Our breakdown of what the SURMOUNT-5 trial showed has the details, and the SURMOUNT-5 tirzepatide trial record lists the design.

The labels tell the same story from separate trials. Zepbound 15 mg produced 20.9% average weight loss at 72 weeks against 3.1% on placebo. Wegovy 2.4 mg produced 14.9% at 68 weeks against 2.4%. A higher 7.2 mg Wegovy dose, approved in March 2026, reached 18.8% at 72 weeks. So wegovy vs zepbound weight loss narrows at the top dose, which no trial has tested against Zepbound.

The share reaching a large loss shows the gap more plainly. On the labels, 56.7% of adults on Zepbound 15 mg lost at least 20% of their body weight, against 30.2% on Wegovy 2.4 mg. Those are separate trials, which is why SURMOUNT-5 matters. It removed the guesswork.

Price & monthly cost

Before discounts, zepbound vs wegovy cost is close, with the White House putting list prices at $1,086 a month for Zepbound and $1,350 for Wegovy (White House, 2025). Cash programs change that. In February 2026, Zepbound's self-pay price started at $299 a month for the lowest dose, more at higher doses (CNBC). TrumpRx, the federal program announced in February 2026, lists injectable Wegovy at an average of $350 a month and as low as $199, and the Wegovy pill from $149, by dose (White House).

Access & compounded options

Both brands require a prescription, and coverage for weight loss varies widely. Compounded tirzepatide and semaglutide are prepared by licensed pharmacies for individual patients. They are not FDA-approved, and FDA does not review them for safety, effectiveness or quality before they reach patients (FDA). Every trial result here comes from the brands.

The shortages that once allowed wide compounding are over: FDA declared the tirzepatide shortage resolved in December 2024 and the semaglutide one in February 2025. Compounding now runs on individual prescriptions, and our guide to compounded tirzepatide in 2026 covers what to ask any provider.

Dosing, pens & the Wegovy pill

Zepbound starts at 2.5 mg a week and rises in 2.5 mg steps, every 4 weeks or more, to 5, 10 or 15 mg. Wegovy starts at 0.25 mg and rises every 4 weeks to 2.4 mg by week 17, then optionally 7.2 mg.

Side effects track the dose steps on both. On the labels, nausea affected 25% to 29% of people on Zepbound against 8% on placebo, and 44% on Wegovy 2.4 mg against 16%. Stopping for side effects ran 4.8% to 6.7% on Zepbound and 6.8% on Wegovy. In SURMOUNT-5, most stomach effects were mild to moderate and came during dose increases.

For anyone weighing the wegovy pill vs zepbound, the Wegovy pill is a 25 mg daily tablet approved in December 2025 that produced 13.6% weight loss at 64 weeks. It is taken on an empty stomach with no more than 4 ounces of water, then a 30-minute wait. Zepbound has no pill form.

Who should choose tirzepatide or semaglutide

Arbor's clinicians treat this as a question of goals, history and budget, and the first choice is rarely the last. Arbor offers compounded tirzepatide injections and compounded semaglutide injections, and the tirzepatide vs semaglutide cost gap there is $149 against $99 a month, flat as the dose rises.

Tirzepatide may fit better when:

  • Result size matters most to you
  • Semaglutide was tried and progress stalled
  • You have obstructive sleep apnea with obesity

Semaglutide may fit better when:

  • You have heart disease, where SELECT showed a 20% lower risk of heart attack, stroke or cardiovascular death
  • Cost matters more than the largest average result
  • You prefer a daily pill to a weekly shot

Some people should take neither. Both labels rule out anyone with a personal or family history of medullary thyroid carcinoma or MEN 2, and both drugs are stopped in pregnancy. Tell your clinician about any history of pancreatitis, gallbladder disease or diabetic eye disease, and about every medication you take, including birth control pills, which Zepbound can make less effective after you start and after each dose increase.

Severe stomach pain that does not ease, with or without vomiting, vomiting that keeps you from holding fluids down, a lump or swelling in the neck, or swelling of the face or throat needs urgent care. For anything else, message your Arbor clinician.

Bottom line on Zepbound vs Wegovy

On weight, the semaglutide vs tirzepatide question has a direct answer: tirzepatide did more in the only head-to-head trial. Wegovy answers with a proven heart benefit, a pill and a lower cash floor. Both carry a boxed warning for thyroid C-cell tumors seen in rodents, and neither is used with a personal or family history of medullary thyroid carcinoma or MEN 2.

A licensed clinician reviews your history before either is prescribed, and if it is not the right fit, it is not prescribed.

Arbor's tirzepatide arrives in a vial, not a pen, so each weekly dose is drawn into a syringe and injected under the skin.
